- An analysis or argument supporting your observations
- Supporting Documents and Evidence
- Copies of all documents related to the exam (e.g., answer sheets, score reports, screenshots if relevant)
- Any prior correspondence with the examiners or the HQ department regarding the exam
- Written testimonies from other individuals, if applicable and relevant
- Statement of Good Faith
- A declaration confirming that all the information provided is accurate and complete, and that the appeal procedure is not fraudulent.
